Hiring Forms for New Employees

Payroll and hiring forms differ based on the type of employment. Please reference the links below to access necessary forms and hiring information.
In addition to the forms linked below if you are not a tax resident of Massachusetts but are:
- An international colleague please review tax information for international employees
- A US resident of one of the states listed on the tax information webpage, please also complete and submit your state's income tax withholding form in addition to the Massachusetts M-4 form included with your hiring forms.
Hiring forms for New, Benefited Faculty and Staff
Your department's personnel coordinator will schedule you to attend a new employee orientation occurring in the first week of employment. In advance of orientation you will receive an e-mail with links to hiring forms (inclusive of the I-9 form referenced above) and information about benefits & benefits effective dates.
Some payroll and benefits forms are unique by the position you will hold. After being scheduled into an orientation you will receive an e-mail with hyperlinks to all of the forms that apply to your new position on campus. Hiring Forms for New Non-Benefited Faculty and Staff
The Hiring Forms for New Non-Benefited Employees contains forms required from all new, non-benefit faculty and staff. Please complete these forms and submit them at the Human Resources Employee Service Center bringing with you original, unexpired I-9 supporting documents.
Hiring forms for New Undergraduate Student Employees
The Undergraduate Student Hiring forms are required from all new undergraduate student employees. You will submit them to the Human Resources representative in the department in which you will be working.
All individuals performing work in the United States are required to present their employer with original, unexpired I-9 supporting documents. Please be certain to bring these documents with you to campus and be prepared to show the original documents to your Human Resources representative.
Hiring Forms for New Graduate Student Employees
The Hiring Packet for New Graduate Student Employees contains forms required from all new graduate student employees. Please complete these forms and submit them at the Human Resources Employee Service Center bringing with you original, unexpired I-9 supporting documents.
Hiring Forms for New PostDoctoral Employees
The PostDoctoral Hiring Packet contains forms required from all new, PostDoctoral employees including the PostDoctoral Health Insurance Plan enrollment form.
Please complete these forms and submit them at the Human Resources Employee Service Center bringing with you both original, unexpired I-9 supporting documents and supporting documentation for any dependents you wish to cover under your PostDoctoral Health Insurance Plan.
