Employee Quick Guide: Steps to Request Family Medical Leave

Employee Goes to FMLA Website


Employee Submits a FMLA Request Form

  • Employee Accommodations and Absence Support Team (EAAS) will determine eligibility. EAAS Team responds within 5 business days with the eligibility notice and next steps

Provide Required Information/Documentation

  • EAAS Team will notify the employee of leave type eligibility, and the required documentation needed
  • Submit required documents via General Submissions.

Employee Accommodations and Absence Support Team Review

  • EAAS Team reviews medical certification and responds within 5 business days with next steps
  • If more information is required, an employee has seven calendar days to have the medical certification updated

Once medical certification is approved or denied, EAAS Team will grant or deny the leave request and provide a Designation Notice to the employee


Return-to-Work if required for your continuous leave of absence

  • A return-to-work form is required prior to a return to work due to your own serious health condition
