GIC eligible employees complete and submit the MA Group Insurance Commission Status/Change Form (GIC "Form 1a") is completed and submitted to UMass Amherst Human Resources when applying to retire from the MA State Employees' Retirement System (MSERS) or the Optional Retirement Program (ORP).
