UMass Amherst recognizes the importance of providing the opportunity for flexible work options to its employees. A successful flexible work arrangement is built on common understanding and clear expectations between a staff member and their supervisor.
Staff members should use this form to request or modify flexible work arrangements for hybrid or remote work schedules. We recommend reviewing the Flex Work guidelines and instructions prior to completion of the request form.
