Instructions - Please Read!
There are a few important steps to follow to ensure you complete your background check form correctly. Don't worry, the process is easy, and we're here to help you. Here's what you need to do:
- Read the first page of the document we've given you and sign at the bottom.
- Go to the second page and fill in the required personal information and your current address.
- For the final step, you'll need either someone from our Human Resources Department OR a notary public to check your government-issued ID and sign the form at the bottom of the second page.
How to Find a Notary Public, or Visit Our Employee Center
Many institutions like banks, credit unions, or libraries offer this service; simply bring your paperwork and your valid and original government-issued ID. There may or may not be a small fee for this service.
Alternatively, you can visit our Employee Center at the Whitmore Administration building at UMass Amherst. Our Human Resources team is there to help you! You can find details about the location and when we are open here.
Types of Valid IDs
Remember, no matter which option you choose, you must bring a valid and original government-issued ID. We can't accept expired or copied IDs. Here are some examples of IDs we'll accept:
- A state-issued driver's license
- A state-issues ID card with your photo on it
- Your passport
- A permanent resident ID card
- A military ID card
Uploading Your Completed Form
Once you've finished filling out and getting your form verified, you must upload it to our online portal, "AskHR." Here's how to do it:
First, visit AskHR, then:
- Tick the box that says, "I am a job applicant or guest."
- Write your First Name, Last Name, and Email in the boxes.
- From the drop-down menu, choose, "My topic is not listed here."
- Then, pick "Talent Acquisition" from the menu that says "Choose a topic."
- After that, choose "CORI Form" from the menu that says, "Choose more detail."
- Now, you're almost done! Click where it says "Choose file..."
- Then attach the completed verified document.
- Finally, click the "Submit" button.
That's it! Your form is uploaded, and you're all set. If you still have questions, don't worry - message us here, and we will get back to you as soon as we can!
