W-2 Wage and Tax Documents
Annual W-2 Wage and Tax Statement
W-2s are available to active employees in HR Direct. W-2s are also mailed to the home address for employees who have not elected electronic-only delivery and for former employees.
Current employees must sign up for Multi-Factor Authentication/Two-Step Login in order to access their W-2s online in HR Direct. Please note, users also need to disable pop-up blockers in their internet browser on this website in order to access/print a pdf of the W-2.
Use HR Direct to Download your W-2
Employees may download the past four years of W-2s through HR Direct by logging in to HR Direct with their Net ID.
For W-2s beyond four years old, please contact AskHR with your request.

Former employees:
If you are no longer actively employed by the university your 2022 W-2 was mailed to your last HR Direct mailing address the week of January 23, 2023. If you have not received your W-2 by February 15, 2023 you can bring a picture ID to the HR Employee Service Center for in-person assistance or open an AskHR case to request a copy (include name and telephone number) Ask HR .
International Employees:
1042-S forms are provided in the SprintTax system. Please reference the International Tax page for more information.
Health Insurance–Related Tax Documents
Two types of tax documents are issued each spring related to health insurance coverage:
1099-HC form (required when filing Massachusetts income taxes): The 1099-HC is a tax document providing Massachusetts residents proof of health insurance coverage. This form is provided by your health insurance carrier directly. Please reference your insurance carrier's website or call the customer service number on your member card with questions about your 1099-HC form. Health insurance carriers are not required to issue 1099-HC forms to Medicare subscribers.

- 1095 forms (issued but no longer required when filing U.S. federal income taxes):
- 1095-C: The 1095-C form reports the months during which an employee was offered health insurance coverage and the lowest monthly premium for coverage offered. Please note - this form does not reflect the cost of actual coverage you may have purchased.
- 1095-A: The 1095-A form reports months of health insurance coverage established through a marketplace (e.g., the Massachusetts Commonwealth Connector). The 1095-A form does not apply to health insurance coverage through the MA Group Insurance Commission (GIC) insurance or the UMass Postdoctoral health insurance plan.
- 1095-B: The 1095-B form reports the months of health insurance coverage under a non-marketplace plan (e.g., coverage under a MA Group Insurance Commission [GIC] or the UMass Postdoctoral health insurance plan). Please reference your insurance carrier's website or call the customer service number on your member card to obtain a 1095-B form.
- 1095-C: The 1095-C form reports the months during which an employee was offered health insurance coverage and the lowest monthly premium for coverage offered. Please note - this form does not reflect the cost of actual coverage you may have purchased.
