Dental Insurance for USA/MTA & MSP/MTA Employees
Dental insurance & vision discount coverage for employees in a position covered by the USA/MTA or MSP/MTA collective bargaining agreement is provided through the MTA Higher Education Health and Welfare Fund.
Coverage Effective Date
Benefited Non-Unit employees may enroll for coverage within the first twenty-one (21) days of benefited employment. If hired on the first day of the month, coverages are effective on date of hire. If hired after the first day of the month, coverages are effective the first day of the following month.
Plan Information
Covered employees can access plan information and download member cards from the plan administrator's website: Health Plans, Inc.. Employees and dependents covered under the MTA Dental Insurance Plan are also covered under the MetLife 20/20 vision discount plan (MetLife Vision discount plan contact: 800-275-4638, press 4, press 2).
Enrolling and Adding a Dependent After Initial Eligibility
You may enroll or add a dependent to your dental plan effective the first day of any future month by returning a completed MTA Dental Insurance Enrollment/Change form to the Human Resources Employee Service Center with proof of the individual's relationship to you (eg, marriage certificate, dependent's birth/adoption documentation).
