How to Submit Your Immunization Records
- Log in to the UMass Patient Portal and go to Action Items > Immunization Records (syringe icon).
- If you may have immunization records in a U.S. state vaccine registry other than Massachusetts, click the blue button that says, "Electronically Request My Immunization Records (Fast & Easy)" and follow the prompts to request your out-of-state records.
- You will receive an email notification when your out-of-state records have been imported by Medicat.
If, after following these steps, you are still missing records:
- Download the Required Immunizations Form and have your healthcare provider complete and sign it, or obtain official immunization records from your provider, school or military sources.
- Log in to the UMass Patient Portal with your UMass NetID and go to Action Items > Immunization Records (syringe icon) to submit your records.
Required Immunizations for New UMass Students
Massachusetts law requires that college students have certain immunizations before coming to school and submit proof of completion. The required vaccines are:
- MMR (measles, mumps and rubella)
- Tdap (tetanus, diphtheria, pertussis)
- Varicella (Chicken Pox)
- Hepatitis B
- Meningitis vaccine
Compliance with the immunization requirements set forth by the Commonwealth of Massachusetts is mandatory for UMass students of all ages.
Failure to submit your complete records by the deadline will result in a hold being placed on your student account. An account hold will prevent you from registering for, adding, or dropping classes until we have your complete records.

Call the Student Immunization Program office, (413) 577-5275, to schedule an appointment to receive required vaccines at University Health Services (UHS).
