2025 Tax Forms: W-2, 1095, and 1099-HC

Taxes

To help you prepare, we've outlined what you need to know about your 2025 tax forms, including the W-2, 1095, and 1099-HC.


2025 W-2 Forms

For Active Employees:

  • Your 2025 W-2 form will be available in HR Direct by the end of January 2025.
  • If you haven't opted for electronic-only delivery in HR Direct, your W-2 will be mailed to your home by the end of January 2025.
  • Employees who have set up Multi-Factor Authentication or Two-Step Login can access their 2025 W-2 online in HR Direct.
  • To access or print your W-2, you may need to turn off any pop-up blockers in your browser. Please reference instructions for Google Chrome users and Microsoft Edge users.

For Former Employees:

  • If you are no longer employed by the university, your 2025 W-2 will be mailed to your last known address in HR Direct by the end of January 2026.
  • If you do not receive your W-2 by February 15, 2026, you can:
    • Visit the HR Employee Service Center in person with a photo ID, or
    • Submit a request through AskHR, including your full name (under which you were paid) and phone number.

Tax filing information for those who are not a United States resident for tax filing purposes

  • Important tax filing information is provided for those who are not a United States resident for tax filing purposes.

1095 & 1099-HC Health Insurance Related Forms

  • 1099-HC forms are required for filing Massachusetts income taxes and are mailed directly from the insurance company.
  • 1095 forms are no longer required to file US Federal Income Taxes.

1099-HC Forms:

  • Important Information Regarding Form 1099-HC
    Form 1099-HC (Health Care Coverage) is issued directly by your health insurance carrier, not by the University of Massachusetts. Employees who need a copy of their 1099-HC or have questions regarding this form should contact their health insurance carrier directly. Health plan carrier contact information appears at the bottom of this webpage.
  • If you were covered by more than one plan in 2025, you will receive multiple 1099-HC forms. Be sure to use all of them when filing your taxes.

1095 Forms:

  • 1095-B: These forms provide information on the months you were covered by your employer's health plan. If you are a benefited faculty or staff member, contact the MA Group Insurance Commission to request a 1095-B form.
    • For Postdoctoral Employees: Blue Cross/Blue Shield of MA will mail the 1095-B directly to those covered under the University's Postdoctoral health plan.
  • 1095-C: The 1095-C form is no longer required for tax filing purposes. Active employees can access their 1095-C form in HR Direct by early March in HRDirect. Current and former employees may request a 1095-C form:
    • In person with a picture ID at the HR Employee Service Center
    • By submitting a request via AskHR **

    • Via telephone: 413-545-0111
