Optional Retirement Program Enrollment Forms

These are the forms required for eligible employees to enroll in the Optional Retirement Program (ORP) in addition to a completed ORP Notice of Eligibility (which is a personalized form provided by Human Resources at the time an employee becomes eligible for the ORP). This packet also contains the form necessary to roll funds from the Massachusetts State Employees' Retirement System (MSERS / pension system) to the ORP.
Please note:
- Enrollment forms must be submitted with proof that a Massachusetts ORP account has been established.
- The ORP and MSERS plan administrators require receipt of the original forms with signatures in ink, forms with digital signatures are not accepted.
