This form is used to request all levels of access to HR Direct.  This includes access to ePAF, self-reporting of time and labor, student hiring and HR administration.  HR Direct access for self-service tasks other than time reporting, like taxes, changing address and viewing your paycheck, are granted for all employees without use of this form.  You will need to download this form in order to e-sign.
