Service Interruption And/Or Task Summary Sheet Form

Service Interruptions are used for notices for planned work that interrupts service to the building's utilities or life/safety equipment. A service interruption may include a required Task Summary Sheet approval. Use the Service Interruption And/Or Task Summary Sheet Form to request approval and to notify the impact to campus audiences. For example, use the form for: No water or no hot water; Steam shutdowns; Planned power interruptions and for any building service testing that occupants may notice (fire alarm with horns, elevator inspections).

Campus Impact Notifications
Use the Campus Impact Notification Form for impacts to the campus community that do not include a service interruption. If you are responsible for work that will reduce or block through traffic, re-route pedestrian traffic on sidewalks or crosswalks, or impede building egress, a request for a Campus Impact Notification is required at least 10 days prior to the start of work. If the impact involves re-routing of pedestrians or building egress, you must first secure a Pedestrian Impact Permit (PIP). Once the PIP has been approved, you can then submit your Campus Impact Notification request.
