Undergraduate Health Withdrawal

Withdrawal Note: Before starting the withdrawal process, please contact your academic dean to discuss whether this is the right decision for you. Taking a health withdrawal erases your grades for the entire semester not just a couple of selected classes.

Withdrawal Form

  • The withdrawal form can be found on the University Health Services page on medical withdrawals and the Dean of Students Office page on withdrawing from UMass, or by emailing bmathieu [at] umass [dot] edu (Brianna Mathieu).
  • Please fill out part 1 completely and send to bmathieu [at] umass [dot] edu (Brianna Mathieu by email) or fax 413-577-7001.

Medical Documentation

Your health care provider must fax a letter to 413-577-7001 Attention: Brianna Mathieu. The letter must include:

  • A medical or mental health diagnosis.
  • The date span the doctor has been treating you (start date–current date).
  • Their approval of your taking a withdrawal. The letter can be written as “I recommend withdrawal for the _____________________ (ex: Fall 2020) semester.” This can be addressed to Dr. George Corey.

Additional Instructions

  • Contact Residential Life to make arrangements to return your key (do this right away, or they will charge you until you move out and return your key) at 413-545-2100.
  • Contact the Financial Aid Department — if you collected any you may have a remaining bill (413-545-0801). 
  • Contact your Academic Dean to inform them of the withdrawal.
  • Contact Patient Services if you have the school health insurance (SHIP) at 413-577-5192 to drop or keep the plan.
  • Contact the Bursar’s Office two weeks after withdrawal has been processed to settle any financial concerns (413-545-2368).

Re-enrollment Instructions

Everyone who withdraws from a semester must do a re-enrollment to return. Please be aware of upcoming re-enrollment deadlines. For an upcoming Fall Semester the deadline is June 30th. For an upcoming Spring Semester the deadline is November 30th. For re-enrollment, instructions please contact bmathieu [at] umass [dot] edu (Brianna Mathieu via email) or at 413-577-5271.
