Undergraduate Health Withdrawal
Withdrawal Note: Before starting the withdrawal process, please contact your academic dean to discuss whether this is the right decision for you. Taking a health withdrawal erases your grades for the entire semester not just a couple of selected classes.
Withdrawal Form
- The withdrawal form can be found on the University Health Services page on medical withdrawals and the Dean of Students Office page on withdrawing from UMass, or by emailing bmathieu [at] umass [dot] edu (Brianna Mathieu).
- Please fill out part 1 completely and send to bmathieu [at] umass [dot] edu (Brianna Mathieu by email) or fax 413-577-7001.
Medical Documentation
Your health care provider must fax a letter to 413-577-7001 Attention: Brianna Mathieu. The letter must include:
- A medical or mental health diagnosis.
- The date span the doctor has been treating you (start date–current date).
- Their approval of your taking a withdrawal. The letter can be written as “I recommend withdrawal for the _____________________ (ex: Fall 2020) semester.” This can be addressed to Dr. George Corey.
