UWW Interdisciplinary Studies Stage 2 Portfolio Agreement

This form is required for all UWW Interdisciplinary Studies students who plan to complete a Stage 2 portfolio after the semester in which they are enrolled in UWW 370. Please discuss your plans for the Stage 2 portfolio with your 370 instructor prior to completing this form, including the third chapter topic(s) / area of learning as well as any additional revision required of the Stage 1 chapters. Aim to summarize your next steps on this form. Once you submit this form, your 370 instructor will need to approve the submission. Stage 2

Please review the items below and sign to indicate that you fully understand and agree to the process. If you have any questions, please contact your 370 instructor.

  • I understand that I will be charged a total of $600 for my Stage 2 portfolio credits, unless I earn 16 total portfolio credits, in which case I will be charged $450 for one Stage 2 portfolio credit.
  • I understand that this charge will appear on my bill after the evaluation process is completed (which may be after the semester in which I submit my Stage 2 portfolio). I understand that this charge does not appear as a course enrollment, and I will need to plan for this cost.
  • I understand that if I enrolled in 370 in the fall, then my completed portfolio is due no later than October 1 of the following year. If I enrolled in 370 in the spring, then my completed portfolio is due no later than March 1 of the following year. If I enrolled in 370 in the summer, then my completed portfolio is due no later than April 1 of the following year.
  • I understand that if I do not meet the Stage 2 deadline, my Stage 1 portfolio will be submitted for credit, and the Stage 2 opportunity will no longer be available to me. The only exception to this is if I have communicated with my UWW 370 instructor at least two weeks before the Stage 2 portfolio deadline and they have approved an extension based on my progress.
  • I understand that I must submit the fully formatted Stage 2 Portfolio, which must include a title page, table of contents, resume, introduction, three chapters, and conclusion, as well as a self-assessment, to my 370 instructor when it is ready for review. My introduction, three chapters, and conclusion may together total no more than 15,000 words or 60 pages, whichever is less.
  • I understand that I must complete this work on my Stage 2 portfolio independently unless I enroll in UWW 396E for 1 credit.
  • I acknowledge that my typed name below is confirmation that I agree to the terms and conditions of the Stage 2 portfolio process.
