Use this form if you are an Undergraduate-Level, Non-Degree student wishing to enroll in a University-Scheduled class that is NOT on the Open Course List. Classes not on this list require instructor permission to enroll.

If the class you wish to enroll in IS on the Open Course List, your enrollment request does NOT require instructor permission. Please use the Open Course List Enrollment form instead.
