The Academic Honesty Office uses DocuSign to collect all forms related to the process. To access the form, click the link to the appropriate type of form and fill out all required information. Once you have finished completing the form, sign it to send it to the next party (either the student for informal resolutions, or the Academic Honesty Office for other forms). Once any form is signed, all parties receive an email notification with a final copy, including the Academic Honesty office.
