Addresses/phone numberspersonal email information and emergency contact information can be updated by students through their SPIRE account.

In order to update your biographical information in SPIRE that you do not have access to change on your own (primary name, social security number, etc.), please use our Name Change/SSN Update Request Form. You can follow either the in-person or electronic steps below.

Note:

  • If you have already filed your FAFSA, please check with Financial Aid to see if a legal name change would have any impact your aid.
  • Current and former graduate students need to change their name through the Graduate School (even if you were previously an undergraduate student). Please contact their office for assistance.

Electronic option

  1. Open the Name Change/SSN Update Request Form.
  2. Choose one of the applicable options to begin the process.
    • Note: if you are a former non-degree/undergraduate student, but your SPIRE credentials are currently active (for instance, you are an active employee), you can use the Current Non-Degree/Undergraduate Student option.
  3. Ready through and follow the instructions that appear on screen very carefully. Once all steps have been completed, submit your request to the University Registrar’s Office.
  4. The University Registrar’s Office will receive an email notification once the form has been submitted. The office will update your SPIRE record and send an email confirmation once complete.

In-Person option

Please come to 213 Whitmore Administration Building with the required documentation needed to process the change. Name changes require legal documentation (court order, driver’s license, valid passport, etc.) and updates to your social security number require your original Social Security Card or a completed Form W-9S.
