Staff Hiring Checklist

The documentation required for staff personnel action forms varies depending upon both the position classification and type of action. The table below lists the required attachments. Please ensure you have the necessary attachments prepared before you submit your hire into the PageUp system.

Academic Affairs Non-Faculty Personnel

USA/MTA and Non-Unit Classified

Hire/Promotion

  • PDF of approved Offer Card from PageUp.

  • Enter PageUp Requisition and PD # in EPAF comments.

Reclassification

  • PDF of HR/Labor Relations Reclassification Letter.

 

All Staff

Data Changes (funding, corrections)

  • Likely none, but may be needed depending on circumstance.  Call if any questions. 

Change in Position Attributes(Supervisor, Building, Etc.)

  • Submit a Salesforce ticket to Compensation with employee name, EMPL ID, position number, changes and effective date.

Retirement

  • Retirement email from Human Resources. 

  • Letter from employee if available. 

Termination

  • Resignation letter, if available. 

  • Other supporting materials, as appropriate

PSU and Non-Unit Professionals

Hire/Promotion

  • PDF of approved Offer Card from PageUp.  

  • Signed Offer and Acceptance form or signed Offer Letter with employment terms.

  • Enter PageUp Requisition and PD # in EPAF comments.  

Lateral Transfer

  • PDF of Transfer Agreement. 

  • Enter PageUp Requisition and PD # in EPAF comments, if transferred through search.

 

03 Temporary Employees

Hire

  • Enter PageUp Requisition and PD # in EPAF comments.
