Use this form to report a lost or stolen University-owned computing device (e.g., departmental laptops, USB drives, cell phones) or personal computing devices that may contain sensitive University data. For any other personal items, please contact the police department where the item was lost or stolen. Please provide us with as much information as possible, but you do not need to fill out every field. Submit a separate form for each device that has been lost or stolen. 

Note: If you have not done so already, after completing this form, please contact the UMass Amherst Police Department to report the lost or stolen device.
