Many health organizations make clinical notes in the electronic health record (EHR) available through patient portal. However, studies have shown that patients are confused by EHR notes. Such confusion may result in unintended increases in service utilization and lead to changes in perceptions that may disrupt patient–provider relationships.  We are developing NoteAid, a multi-component, natural language processing (NLP) system that translates medical jargon into lay terms and provides definitions and links to related educational material from trusted resources. We are building a large resource of medical jargon and definitions, developing innovative NLP approaches for ranking jargon that are important to patients, and building evaluation methods to assess the readability and patients’ comprehension of EHR notes.Our evaluation results show that both patients and physicians like the NoteAid system, which improved self-reported comprehension of EHR notes.
