All injuries, exposures, and spills must be reported to EHS (413-545-2682) promptly. A Lab Incident Report should be completed and filed with EHS within 48 hours of all incidents. 

Individuals who are paid to perform duties in a lab must also submit a completed Notice of Injury form to Human Resources within 48 hours of the injury/illness. Every injury or work-related illness must be reported, regardless of whether it might seem relatively minor or insignificant.

Accidents can occur even after conducting thorough risk assessments and implementing appropriate control strategies. Every incident can be used as teaching opportunities to understand what went wrong and where we might improve. The Incident Report Database includes past lab incidents for the community to understand and learn from so that we can devise and implement safer alternatives.
