To initiate a UHS eating disorder evaluation, schedule an appointment with a physician and nutritionist at (413) 577-5101.

Your evaluation will begin with a 45/60-minute meeting with a nutritionist who will discuss your diet history with you. You will then meet with a physician for a 30-minute medical evaluation.
