Under 18? 

You will need to email this Liability Release Form to your parents for them to complete, sign and return to you. Once you receive the signed form, email it to Intramural Staff Nicholas D'Amato (ndamato@umass.edu) with the sport and name of the team you wish to join. NOTE: If you want to digitally sign this form, you will need to download it and sign it in Adobe Acrobat or Adobe Reader. It cannot be signed if opened in the browser.
