To complete your Art Minor you must submit:
- This form, printed and filled out
- The Declaration of Minor form, available from the Registrar’s office, printed and TOP SECTION (section “A”) ONLY completed (leave section “B” blank)
- A printed copy of your unofficial transcript from SPIRE, with required Art and Art History courses highlighted
Submit all above materials to the Art Department main office, SAB 218 before the end of your final semester
