UWW is a part of UMass, but it feels like a small school and that you're really somebody at the school and not just one in a million. I like having such small classes. I like my advisor. I like that everybody in my classes has a lot in common with me—they're my age, they don't take school for granted and we share a great deal in getting our degrees from UWW as a group

Forms used by UWW Students

Note: The Excel and Word forms have macros embedded in them to automate buttons and calculations. Please enable macros if requested to do so when you download the form.

Individualized degree plan form (in Excel) ms excel icon: to be developed within UWW 196G Frameworks of Understanding and submitted for approval to the UWW review team.  For use only by UWW students in the Professional Studies program. (See the list of potential individualized degree concentrations and other degree planning materials.) You do not need to know how to use Excel to use the degree plan, but you must have a copy of MS Office. . Note: This form contains macros for the buttons to work, so you may have to change Excel security settings to medium under Tools/Options, and then say yes when asked to enable macros on loading. You may choose to work with a straight text degree plan form, but it will not automatically format and calculate credit totals in residency, concentration, etc.

Elementary Licensure Program degree plan ms word icon: to be used only by students enrolled in the UWW Elementary Licensure Program.  

Special transcript evaluation form ms word icon: to be used for credit evaluation of workshops, training courses, and other forms of structured learning not previously credited by a college or university. Instructions and details of what constitutes acceptable documentation are included with the form.

Prior learning transcript form ms word icon: to be submitted with the prior learning portfolio. The first page includes the instructions and the second, the form itself. This form lists key experiences discussed within the portfolio and the areas of learning for which the student seeks credit. Once the transcript statement is signed by the evaluator, the information on this form will appear on the student's transcript.

Independent Learning Contract: ms word icon to be completed with the UMass Amherst faculty member sponsoring the independent learning project or internship.

Continuing & Professional Education Intent to Graduate Form (scroll down on page): pdf icon to be submitted to Continuing & Professional Education with payment for the graduation fee during the semester in which the student will graduate.

Other Continuing & Professional Education forms: All Continuing & Professional Education forms can be downloaded from the Continuing & Professional Education web site. These include forms for withdrawal, instructor permission to enroll, late add, prior approval for courses taken elsewhere, credit overload, etc.

University Registrar forms: Most of these forms and resources will not apply to UWW students registered through Continuing & Professional Education. The few UWW students still registered through the University Division, however, should use these forms rather than those on the Continuing & Professional Education web site. Some materials downloaded from the Registrar's site may be of interest to all UWW students, such as:
