You have a few choices in how to obtain a copy of the Performance Evaluation. If you have Adobe Acrobat Reader, you can go to the Library section of Human Resources, click on USA/MTA Bargaining Unit Contract and print pages 91-94. To download a copy of Adobe Acrobat Reader (always a good idea), click here. You can call Human Resources at 545-0862 to request a copy or you can use the orange copy that should be automatically sent to our supervisors.
