Open Enrollment for Health Care Spending Account & Dependent Care Assistance Programs

This is your once annual opportunity to enroll in the pre-tax Health Care Spending Account and Dependent Care Assistance Program for calendar year 2008!

Health Care Spending Account (HCSA)

HCSA allows you to pay for certain health care expenses on a pre-tax basis. Eligible expenses include:

 Many over-the-counter medications including antacids, allergy medicine and pain relievers,

 Physician office and prescription drug co-payments & deductibles,

 Eyeglasses and contact lenses not covered by your health or vision insurance,

 Orthodontia and dental expenses not covered by your dental insurance,

You may contribute between $500 and $2,500 on a pre-tax basis in calendar year 2008.

Free Debit Card: If you enroll in the HCSA you will receive a debit card so you may immediately use funds in your account rather than submit a claim. Additional cards are available for $5.00 each for this year.

Dependent Care Assistance Program (DCAP)

DCAP allows you to pay for qualified dependent care expenses on a pre-tax basis, reducing your federal and state income taxes. Eligible expenses include day care, day camp, after school programs and nursery school. You can contribute up to $5,000 on a pre-tax basis in calendar year 2008.

Qualified expenses are for dependents under age 13.

The monthly, pre-tax participation fee for one or both plans is $3.75.

Before you enroll, calculate your eligible expenses for 2008 - remember that money not spent before March 15, 2009 will be forfeited to the plan. The amount you elect is deducted proportionally from your paycheck over the calendar year.

Current participants are encouraged to enroll on-line at www.enrollone.com.
