Your first visit
If it’s your child’s first visit with us, please arrive ½ hour before the scheduled appointment to complete the registration process.
Bring all immunization records and any available medical records with you to this visit.
Save time by downloading the Pediatric Assessment Form from this website; bring the completed form to your appointment.
Routine care at UHS is by appointment; call (413) 577-5101. Established patients should arrive 15 minutes early for appointments, to allow for any necessary testing or paperwork.
Walk-in care is available for minor illnesses and injuries that can't wait for an appointment;. See the home page of this website for current hours. Our medical professionals will offer advice and see your child if necessary; an on-call pediatrician is also available as needed. Outside of operating hours, UHS offers telephone triage and consultation services.
UHS is not a hospital and does not have an emergency department. In a life-threatening emergency, call 911 for ambulance transportation to a hospital.
