(1)   Application for Admission to the Major in Public Health Sciences for students currently enrolled at the University. Admission Form for Public Health Sciences

Incoming students must apply for admission using the undergraduate admissions process at http://www.umass.edu/admissions/apply/

(2)   List of Social Science Track courses or Science Track courses.

PUBHLTH courses must receive a "C-" or better

(3)   A current transcript (this can be an "unofficial" transcript printed from SPIRE).
