SBS Advising Center: Forms
For your convenience, many of the required forms for undergraduates are available online. Please print them out, fill them out (writable pdfs require Adobe Acrobat Professional), and bring them to the SBS Advising Office for signatures, etc. Be sure to follow instructions precisely. All forms are available in paper format at the SBS Undergraduate Advising Center, 128 Thompson Hall. The Advising Center is open from 8:30 a.m.–5:00 p.m., M–F, except on holidays. If you have any questions, come to the center. We are there to help you.
Global Education Option Declaration Form [writable pdf]
Global Education Course Substitution Form: Students who wish to apply Five-College, UMass Amherst, or transfer courses that are not on the Global Education Regional Study Option Approved Course List may do so with the approval of your Global Education Advisor. Please visit the SBS Advising Center, 128 Thompson Hall to obtain the triplicate form that requires course information and your Global Education Advisor’s signature.

Late Course Add Petition [writable pdf]
The completed “Late Course Add Petition” must be returned to 128 Thompson Hall along with a signed “Course Change Request” form (available from the Undergraduate Registrar’s Office, 209 Whitmore Administration Building, or the SBS Advising Center, 128 Thompson Hall).
Late Course Withdrawal Petition [writable pdf]
The completed “Late Course Withdrawal Petition” must be returned to 128 Thompson Hall accompanied by a signed “Attendance and Performance Verification Form” and a signed “Course Change Request” form (available from the Undergraduate Registrar’s Office, 209 Whitmore Administration Building, or the SBS Advising Center, 128 Thompson Hall).
