This Pre-Award and Pre-Establishment of Account form is provided in two Word 97 formats. The Interactive form can be filled out on-line, the Static form must be printed out and filled in manually. If you do not have this version of Word on your computer you will not be able to open either file and use it. You may be able to open the PDF version (check Help on the previous page for more information on how to easily access PDF files). All of the files below may be downloaded and saved on your C: drive. Once the original is on your C: drive, the file can be reused -- you don't need to download it again. After filling in the form or changing the file in any way, remember to save it under a new name.

To use the Interactive Word form, click on the interactive form link, save the file to disk when prompted, extract it by clicking on the file name (peaform.exe) in Windows Explorer and then open the extracted file in Word. The extracted file is a form template and is named peafrm.dot. It may be opened like a regular document from within Word. Tab between fields to enter information.
