There are three (3) different types of files listed here:
HTML - To print simply click on the form and print it from your browser
PDF - To print you must have Adobe Acrobat Reader
loaded on your PC or Mac. Go to Software
Support if you don't currently have this software. It is
free and works on all platforms (PC and Mac). Once the file is viewed
in Acrobat Reader, simply save it to your computer and/or print it to
your printer. Printing forms with this method produces a better, cleaner
document for submission and does not have the internet URL address
at the top.
Word or Excel which may be filled in on-line. (Page 1 is in MS Word 97 and Page 2 is in MS Excel 97.) When you click on the file, you will be asked whether you want to "Open it" or "Save it to disk".
If you select "Open it" the application will launch and bring you to that file. You can work on and print the file at this point. The next time you want the blank form you will have to download it again.
If you choose "Save it to disk", you will prompted to indicate where the fille will be saved on your local or network hard drive. Once the file is saved, open it in Word or Excel and fill it in. Hint:
Remember to rename the filled-in file before you save it or the blank form will be over written.
