OGCA Internal Processing Form (IPF)

Please note that the 4th page of the IPF is new and required for all proposals.

You must have Microsoft Word installed on your computer in order to load this form. Please click on each field individually and do not Tab between fields. You may partially fill in the form and save it to your hard drive, as a template for future use. Please note, the document is now protected which allows the boxes to be checked off. The down side to this is the links will not work. If you want the links to work, you can unprotect it with a password of "123".
