The Institutional Animal Care and Use Committee (IACUC) has converted to a new on-line animal use approval system called e-Protocol IACUC. From March 1, 2009, please submit animal use protocols for IACUC review through this system, with the exception of custom antibody production. Note that submissions of new animal protocols, including protocol renewals, on the old paper forms will not be accepted after June 1, 2009.
In order to access e-Protocol IACUC, you will need to contact the IACUC office with the following information: Name, Department, Title, e-mail and NETID. (NETID is the login that you use to check your e-mail through UMAIL).

IMPORTANT - You will receive an email from e-Protocol IACUC giving you a temporary password for the system. Please follow instructions in the email immediately to set your own password.
The following forms will not be accepted after June 1, 2009 for IACUC protocol submission:
- Section I - General and Introductory Information
Section II - Rationale and Justification for the Use of Animals
- Section III - Qualifications and Training of Personnel
- Section IV - Description of Animal Use
- Section V - Statements and Signatures
The following must continue to be submitted on the paper form:
- Addenda to existing paper based protocol
- Appendix A - Animal Use Addendum to a Protocol on the old protocol form
- Custom antibody production protocol (either by UMass Amherst antibody production facility or through an off-campus vendor).

Amending a protocol

Revisions to protocols approved in e-protocol must be submitted through e-Protocol IACUC by revising the existing protocol.

Annual Report
e-protocol IACUC
Annual updates to your protocol approval are submitted through e-protocol. You will receive an automatic email reminder from e-Protocol IACUC when the annual report deadline is approaching.

• Appendix U - A hard copy of the Annual Protocol Update/Protocol Continuation. You will be informed by the IACUC Office when your annual report deadline is approaching.
