Templates for Animal Cage ID Tags
Several different styles are offered.
If you do not already have your own descriptive cage tags, check those offered here, and choose the type that is best suited to your animal cage ID needs.
If you have an animal cage ID tag format that you feel will be useful to others, please send to us for posting here.
If you do not find a format that fits your needs, please contact the Animal Care Office.
Note that hand-stamped cards are still available through Animal Care technicians in the different animal care and research areas.
Surgery and Post-op Record Forms
- Note that the surgery record sheet and post-op care sheet are designed to be used together. You can either print the form, which contains both, onto 11x17 paper. Or you can select the two areas separately (surgery and post-op) and print on 8x11. If you use a three-ring binder, for example, the surgery record is hole punched on the right margin and placed on the left side of the notebook. The post-op care is punched on the left and placed on the right, so one can "follow" an animal's record directly across the two pages from the start of surgery to the end of the immediate post-op time.
