Applicator License Training Workshop
There are three ways to register for the workshop.
1. For a 2014 Training Workshops information and registration form that you can mail in with your registration check click here.
2. We can also accept credit cards for payment of the workshop fee ($140.00/person) using an online registration process. There will be an additional online service fee of approximately $11.00 for each registration. Before you register for the workshop please be advised that you are required to have the 2014 pesticide exam study manuals (Core Manual 3rd edition, 2014 Massachusetts Core Supplement, 2000 Massachusetts Pesticide Control Act & 2010 Mass. Pesticide Regulations). You should order these manuals before you sign up for the workshop. Call the UMass Extension Bookstore at 413-545-5537 to order the study manuals.

Please select the location and date of the workshop you wish to attend. Please consider selecting a workshop that is held at least 1 week before your exam.:
The registration fee is $140.00/person. (This fee only covers the cost of the workshop. There is an additional fee of $11.00 if you choose to pay by credit card online. Pesticide exam study materials and exam fees are not included. Please order your pesticide exam study materials separately as soon as possible)
Make the check or money order payable to University of Massachusetts. Governmental Purchase orders may be submitted via fax (413)545-5858. Tax ID Number is 043-167-352.
Print (2) copies of this form, keep one copy for your records and mail the other copy with your check or money order to:
