All current U.S. Department of Labor (DOL) Family and Medical Leave Act (FMLA) forms (with one exception) expire July 31, 2018; the Certification of Health Care Provider form for an Employee's Serious Health Condition on the DOL's FMLA website still has a May 31, 2018 expiration date. All current FMLA forms have been uploaded to the UMass Human Resources Forms Library and also appear on the DOL's FMLA website.
DOL must sumbit forms to the US Office of Management and Budget (OMB) for review every three years. Historically expiration dates are extended monthly until new forms are approved by the OMB.
When processing leave requests for employees who may be eligible for protections under the FMLA please use the forms on the DOL's FMLA website as they exist at the time you are processing the leave.
