Please make a double-sided copy of the eligibility form, then fill in the blanks and have it signed as indicated. A paper form must be submitted with original signatures by the deadlines indicated.

DEADLINE to complete and submit all requirements for your degree:

                              February Degree - December 15th (or next working day if December 15th is a weekend or holiday)
                              May Degree - April 15th (or next working day if April 15th is a weekend or holiday)
                              September Degree - last working day of August (usually August 31st)

Doctoral ORAL EXAMINATION announcement deadline: All requests to schedule Doctoral Oral Examinations must be received by the Office of Degree Requirements one month prior to the exam date.
