Dig Safe Video and Certification Form

Anyone engaging in operations on the campus of UMass Amherst that is considered to be excavations per the MA Dig Safe Law M.G.L c. 82, §§ 40 must watch the following training video prior to such work. Under this law “Excavation” means an operation for the purpose of movement or removal of earth, rock or the materials in the ground including but not limited to digging, blasting, auguring, backfilling, test boring, drilling, pile driving, grading, plowing in, hammering, pulling in, jacking in, trenching, tunneling, and demolition of structures.

After watching the video, please complete and submit the form below. You will receive a certificate of training completion by email that is valid for two years. Please renew your training every two years by returning to this form and re-submitting it.

Statement-2 *

I understand that an excavation permit – for any excavations exceeding 36” in depth – must be submitted to UMass 3-working days prior to the excavation, and that I will comply with all MA excavation laws.

Statement-3 *

I understand that all Dig Safe markings shall be maintained as excavations proceed, and that it is the excavating contractor’s responsibility to maintain these marks.

I understand that if a re-mark is needed, another Dig Safe request shall be made.

Statement-5 *

I understand that if the scope of work has changed, a new Dig Safe request shall be made.

Statement-6 *

I understand that the excavation contractor shall meet with the UMass Dig Safe Coordinator to review the site prior to excavation.

Statement-7 *

I understand that the contractor shall review the scope of the day’s work, and the necessary site drawings on a daily basis to ensure that any utilities are identified; the utility map shall be kept at the site for quick reference.

Statement-8 *

I understand that notification to the UMass Dig Safe Coordinator and appropriate Dig Safe partner MUST occur upon discovery of conditions that vary from the provided markings.

I understand that hand-digging shall be done within 18” on either side of the utility, or also as needed.

Statement-10 *

I understand that if a utility line is hit, operations must cease, and contact made to the Facilities & Campus Services Solutions Center (413) 545-6401, www.umass.edu/facilities, and if the hit presents a public safety concern, to the UMass Amherst Police Department (413) 545-3111, www.umass.edu/umpd. Do not remove excavation equipment from the site or alter the site in any way unless necessary to preserve life and property.
