It's Your Best Move! Founded in 1977, Fallon Community Health Plan provides health care
services designed to meet the unique and changing needs of all we serve.
FCHP is the only health plan in Massachusetts that is both an insurer
and provider of care.
Our insurance and self-insurance product portfolio includes a variety of
group, non-group and individual health plan options (HMO, POS and PPO,
as well as Medicaid and Medicare Advantage plans) featuring flexible and
innovative benefit designs. Our uniquely developed provider networks
offer high-quality, cost-effective, coordinated care and give our
members access to physicians and hospitals throughout Massachusetts. We
also offer a broad spectrum of health and wellness services and programs
to ensure our members, at every stage of life, remain as healthy and
productive as possible.
As a provider of care, FCHP operates Summit ElderCare, an advanced and
completely integrated program that offers seniors and their caregivers
an alternative to nursing home care.
Our continued commitment to deliver high-quality health care and
exceptional customer service has earned FCHP consistent ratings as one
of the nation's top health plans. Both our commercial HMO and Medicare
Advantage plans have been awarded "Excellent" accreditation by the
National Committee for Quality Assurance. We continue to receive "Best
in Class" designations from HEDIS(r) for a number of quality measures,
including pediatric immunizations, well-child visits and prenatal care.
For 2007, our Medicare and Medicaid plans were named #1 in the country
by the National Committee for Quality Assurance (NCQA) and the U.S. News
& World Report.*

Fallon Community Health Plan in the Community:
At Fallon Community Health Plan, we think community service is so
important that we encourage our employees to volunteer. Working with
many nonprofit organizations, Fallon Community Health Plan invites
employees to volunteer one day per year, on company time, in programs
that improve the health of the elderly, youth or economically
disadvantaged in our service area.
Our employees have volunteered at such organizations as:

- Meals on Wheels

FCHP financially supports organizations that promote health care and
social services across our service area through our Community Benefits
Program.
We are committed to making our communities healthy. Since 1977, FCHP has
worked to improve the quality of life and the health status of
individuals by offering access to high-quality, affordable medical care
and service.
FCHP will work cooperatively with health care and community service
organizations, as well as state and federal agencies, to lead the
creation of innovative health care solutions, to seek healthy outcomes
and to improve access to health care services. FCHP will make resources
available to community organizations as appropriate in pursuit of these
goals.
This year, FCHP will distribute a limited number of grants to eligible
organizations to support new or existing programs that provide health
care or social services to two target populations:
- Children in the first three years of life
- At-risk youth between the ages of 12 and 19
