Final Report Form

CONGRATULATIONS!

Once you have received a Mini or SP/E grant from the UMass Arts Council, you are now required to complete the Final Project Report Form. The Final Project Form is an important way for the Council to assess the success of events which it supports, and to determine the steps we need to take to improve our grant program. Although all applicants receive a copy of this form with their notification letter, we also have made it available here to download. Here are a few tips to help you complete your form:

1. Download a copy of the Final Project Report Form in Microsoft Word .rtf Format. OR the Final Project Report Form in Adobe Acrobat Reader .pdf Format

2. Fill out the form completely and attach any publicity materials from the event (press releases, newspaper clippings, ads in papers, print outs of web pages, etc.)

3. Check over your budget figures carefully, and remember that any surpluses must be returned to the Council.

4. Mail or deliver your completed form within three weeks of the conclusion of your event to: UMass Arts Council; 10 Curry Hicks; University of Massachusetts; Amherst, MA 01003.
