Eligibility:
You may qualify if you are 19 years old or older, are United States citizen or legal resident, and earn less than 300% of the federal poverty level. For one person, this means earning no more than $29,412 a year. For a family of 4, this means earning no more than $60,012 a year. As of now, most people who get health insurance through a job or other public program do not qualify for Commonwealth Care.
Fees:
Commonwealth Care is free for people who are below or at the federal poverty level. People who earn between 100% and 300% of the federal poverty level pay an amount that is based on their income and choice of health plan.
Intake Process:
To apply for Commonwealth Care, fill out a Medical Benefit Request (MBR) form. You can download and complete the application on line, then follow the instructions for mailing. Once your application has been processed, you will receive a letter from MassHealth telling you which program you may qualify for. The kind of health coverage you get depends on your family size, income, and other circumstances. You will be given the most complete coverage that you qualify for.

Commonwealth Care is an insurance program for uninsured individuals with incomes that fall within certain guidelines and who meet other qualifications. With Commonwealth Care, you choose a health plan and you choose your own doctor. The benefits of Commonwealth Care include regular check-ups, treatment when you are sick or injured, prescriptions at your local pharmacy, vision care, mental health or substance abuse treatment, and, for some members, dental care. Commonwealth Care plans are offered by Boston Medical Center (BMC) Health Net Plan, Fallon Community Health Plan, Neighborhood Health Plan and Network Health.
Commonwealth Care pays for:
-Hospital stays
-Check-ups with your regular doctor, including x-rays and labs
-Emergency room visits
-Eye care, including an exam and glasses every 2 years
-Prescription drugs
-Mental health and substance abuse services
-Care from specialists (such as surgeons or gynecologists) who work in your doctor's office, community health center, or hospital
Commonwealth Care may also pay for other health care services. These depend on your income and choice of health plan. For instance, people with income below federal poverty level get dental care.
