DESCRIPTION OF SERVICES: The At Home Programs has been developed to respond to the ever-increasing desire of persons to live, work and spend quality time in their homes. The variety of services offered reflects a broad range of individual and organizational needs. From Home-Based Services, to Home Business Development, The At Home Program is available to assist in all aspects of people's lives within their home environment. At Home Services Provides: -Community Service Network: links people requesting home and nursing home based services with professionals providing these services. Services include: companions, helpers, shoppers, tutors, meal planners, counselors and more. -LifeCare Support and Management: a full-service program assisting families in the specialized care of individuals requiring ongoing care options. This program assigns a professional care manager to oversee the often complex and ever-changing needs of individuals, allowing their families to use time for other aspects of their lives, feeling secure that their loved one's needs are fulfilled. -Geriatric and Pediatric Services: designed to meet the needs of programs and families. From assisting in the design of a wheelchair accessible, or Alzheimer's safe home, to finding age-appropriate toys and learning tools for a child with special needs. -The Sunderland Center for Positive Change: offers a variety of programs including, support groups, tutoring, counseling to individuals and families in search for positive growth and change in their lives, family nights and workshops and training designed for both the professional and para-professional group.
