Exiting Agreement between the Entomology Graduate Program of Plant, Soil & Insect Sciences, University of Massachusetts and Graduate Students who have not completed their Degree Requirements.

I, _________________________ enter into this agreement with the full knowledge of the potential consequences and take full responsibility to see that all unfinished requirements, either of the Department or of the University, are completed by ___________________________(date).

I recognize that I have taken this action to leave the Department without completing Degree Requirements against the advice of the Department. If this timetable is not met, I realize that the Department need not request any future extensions of my graduate program beyond the above date.

Because this action alters the normal sequence of events leading to my graduation, the normal timetable for faculty response to those unfinished degree requirements as established in the Entomology Graduate Handbook may no longer apply.
