Read Site Descriptions, and list any specific rotation sites for which you have a preference. Because actual rotation sites and/or combinations of sites always vary somewhat from year to year, it is important that you include a brief explanation of why you are listing particular sites on this sheet. If there are any sites at which you prefer not to be placed, please note that in the "comments" section, along with an explanation. Return with application. Please note that preferences listed are not used to make acceptance decisions, and information from this form is only part of that used to assign matched students to available sites. Student preferences are taken into account, but we do not guarantee placement at specific sites. We also cannot guarantee that interns will not be placed at sites they "prefer" not to go to.
