Applications Due: February 16, 2010
- Application form: ADA's 2005 Standardized Supervised Practice Program Application
- Application fee: $50.00 check or money order, payable to U.Mass. Division of Continuing and Professional Education.
- Letter of application: In no more than two (2) pages, tell us: why you want to become a Registered Dietitian; why you want to attend the Dietetic Internship at the University of Massachusetts Amherst (rather than other supervised practice programs); about a situation in which you needed to work independently, and comment on the skills you used for successful completion. The letter MUST be typed or word-processed in an easy-to-read font (e.g. Times, Courier, Arial, Helvetica) in a size at least 11 point.
- Official transcripts from all colleges or universities attended through graduation or through first semester of senior year if applying before graduation. A list of courses to be completed prior to graduation must be attached if applicable. All transcripts must show descriptive titles. If only course numbers are shown, provide a catalogue description also.
- Official verification of completion of ADA-approved or -accredited Didactic Program in Dietetics (DPD), or a declaration of intent to complete degree and ADA academic requirements, obtained from your DPD Program Director.
- Three to five letters of reference from appropriate professionals (e.g. faculty, academic advisor, employers), on ADA's Standardized Recommendation Form. At least one reference must be from an employer (either paid or volunteer). All letters should be submitted in sealed envelopes with the writers' signatures across the seal. Note: We do not require a separate letter from your references, as long as they are able to adequately offer their assessment of your abilities to be a successful Dietetic Intern on the ADA Standardized Recommendation Form.
- “Site Preferences Sheet”: Although this sheet will not be used to make admissions decisions, it is helpful for assigning matched interns to rotation sites as early as possible. Therefore, it must be sent in with the other application materials. Click here for a copy of the site preferences sheet. Be sure to include site preferences for Clinical, Management and Community rotations, and WHY you are interested in these sites.
Keep a copy of your application for your own records, as application materials submitted to this program will not be returned to applicants.
