Withdrawing During a Semester
If you are withdrawing from the University because of:
• Personal, Academic, or Financial Reasons then you must initiate the withdrawal at your Academic Dean’s office. If you do not know who your Academic Dean is, visit http://www.umass.edu/registrar/gen_info/findmydean.htm
• Health Reasons then you must initiate the withdrawal by contacting the University Health Services Medical Director Office at (413) 577-5211 or http://www.umass.edu/uhs/accesstocare/withdrawls/
• Military/Active Duty then you must initiate the withdrawal by contacting Veteran Services at (413) 545-0939 or the Dean of Students Office at (413) 545-2684. Please note that you will need to provide a copy of activation letter or orders.
Please note that all withdrawals must be signed by you, the student. Any other signature will not be accepted and the withdrawal will not be processed.
Withdrawing at the End of a Semester
If you are withdrawing from the University AFTER the current semester ends, but before the first day of classes for the following semester, complete an End of Semester Withdrawal Card and return it to the Undergraduate Registrar’s Office:

You can also notify the Registrar's office in writing of your withdrawal. Please include your Student ID number and your signature.
Readmission to the University
All students planning to return to the university after withdrawal must file a Readmission Application with the Office of the Registrar.
The form is available at
http://www.umass.edu/registrar/forms , or in the Registrar's office, 213 in the Whitmore Administration Building.
