Privacy Request

Students wishing to privatize their directory information should complete a Privacy Request Form. You can download the form, complete it and send it to the Dean of Students Office or come into the office to fill it out and ask any questions you may have.

Directory information includes a student's:

Name
Local address
Home address
Telephone numbers
Email address
Dates of attendance
Major
Date and place of birth
Acknowledgement of participation in officially-recognized University activities and sports
Weight and height of members of athletic teams
Degrees, certificates and awards
Student employment status
Most recent previous educational institution or agency attended
