Dean's Certification/Conduct History Clearance
Dean's Certification/Clearance is verification that you, as an UMass student, have or have not violated any University policies or been subject to any disciplinary actions or proceedings. Dean’s Certification is usually required by graduate and professional schools, state bar associations, government agencies, or independent agencies when applying for admission or employment. Most of these institutions will provide you with a form for you to fill out and the Dean of Students office to complete. Our office will fill out the form and attach our own clearance letter and mail it directly to the institution.
Please use the Clearance Request Form located in Spire when seeking to obtain a personal copy of your conduct history or to authorize the Dean of Students Office to provide a copy of your conduct history to external agencies. To submit a request, log into SPIRE and navigate to: Main Menu > Student Home > Dean of Students Request.
Conduct Case Records
- Please use the Conduct Case Records Request Form located in Spire when seeking to obtain specific file(s) related to an incident that initiated a conduct case involving you as either the Complainant or Charged Student. Please note, we cannot provide copies of police reports. To submit a request, log into SPIRE and navigate to: Main Menu > Student Home > Dean of Students Request.
